The hospital-home transition care aims to effectively maintain the continuity of home care. When well implemented, it positively interferes with the continuity of treatment, as it offers the necessary support to the elderly and their families, avoiding decompensation, adverse events and readmissions. The aim of this study was to analyze the hospital-home transition care practiced by nurses to elderly people as available in the scientific literature. An integrative literature review was carried out between the months of February and March 20212020, through searches in scientific databases using pre-defined inclusion and exclusion criteria. Twelve articles were found and analyzed through exhaustive reading of the content, in order to apprehend the authors' idea on the subject. Great challenges were found to align the work of nursing professionals, who lack information, with regard to the execution and resources that enable the practice of transitional care. The consequence of these mishaps is insecurity: nurses do not find security to perform their functions with regard to the transition of care, family members feel neglected and unprepared to provide care at home and elderly people experience total helplessness. It is concluded that transition care is performed incompletely and without assistance from nursing professionals.
